Southern Trust launch new drive to help patients with diabetes
Southern Trust has launched a new drive to help patients with diabetes to better manage their condition if admitted to hospital.
In the Southern area there are around 16,000 children and adults with diabetes. Whilst not always directly related to their condition, currently one in six people admitted to hospital has diabetes.
District Nurse Manager, Katrina Loughran and Diabetes Specialist Nurse, Aine Kelly worked together as part of a regional Quality Improvement project to encourage patient involvement to ensure a safe discharge of patients on insulin and to improve the transition of these patients into the community setting.
Launching the leaflet Aine Kelly, Acute Diabetes Specialist Nurse for the Southern Trust said, “Diabetes is one of the most challenging chronic conditions facing our population.
“People with diabetes are at risk of significant health problems like renal disease, sight problems or cardiovascular disease, so while not always directly related to their diabetes, are more likely to need a hospital admission.
“Maintaining good glucose control during any hospital admission is so important for people with diabetes, helping to promote recovery, reduce the risk of potential complications and preventing the need for a longer stay than expected,” she added.
Katrina Loughran, District Nurse Manager added, “It is recommended that patients with diabetes are kept fully involved with decisions relating to their condition whilst in hospital to promote safe and effective care during their stay and following discharge.
“Whilst our hospital team will support patients to safely manage their insulin or any other medications, we hope this leaflet will help inform, encourage and support patients in hospital to remain independent and to be involved with the management of their diabetes at home.”
Any patients experiencing complications relating to their diabetes during their stay at Daisy Hill or Craigavon Area hospitals can be referred to the Inpatient Diabetes Specialist Team.
